Your hair transplant results depend just as much on proper aftercare as on the surgery itself. Follow this comprehensive guide to protect your grafts, speed your recovery, and achieve the best possible outcome.
Recovery timeline
Rest & Protect
Sleep in a semi-upright position (45 degrees) using 2-3 pillows or a travel pillow around your neck. This reduces swelling and prevents accidental contact with the transplanted area. Take prescribed pain medication before bed. Mild oozing or pinkness from the donor area is completely normal.
Early Healing
Small crusts and scabs will begin forming around the transplanted grafts - this is a normal part of healing. You may experience mild swelling on the forehead that gradually moves down toward the eyebrows. Continue sleeping elevated. Avoid touching, scratching, or picking at the recipient area. Apply saline spray as directed to keep grafts moist.
First Wash & Follow-Up
Your first clinic-guided hair wash typically occurs on day 3-4. By the end of week one, most crusts and scabs should have gently washed away. You will attend a follow-up appointment where Dr. Shah will examine the grafts and donor area. Donor area redness (if FUE) will still be healing. Most patients can return to desk-based work by day 5-7.
Return to Normal Activities
Swelling has fully resolved. The recipient area may appear pink or slightly red, which is normal. You can begin washing your hair more normally with gentle, circular motions. Most social activities can resume. The donor area is largely healed. Avoid direct sun exposure on the scalp.
Shock Loss Phase
The transplanted hairs will begin shedding - this is called "shock loss" and is completely expected and temporary. The follicles remain alive beneath the skin and will re-enter the growth cycle. Do not panic. This is a natural phase that nearly all transplant patients experience. Continue any prescribed medications (finasteride, minoxidil) as directed.
New Growth Begins
Fine, thin new hairs start emerging from the transplanted follicles. Growth may appear uneven at first - some areas grow faster than others. The new hairs may initially be lighter or finer than your existing hair. This is normal, and they will thicken and darken over the following months.
Visible Improvement
Approximately 50-60% of the transplanted hair has grown in by this stage. You will notice a significant improvement in density and coverage. The hair continues to thicken and mature. Many patients report feeling much more confident at this point. A follow-up appointment with Dr. Shah is scheduled around this time.
Full Results
The final result of your hair transplant is visible at 12-18 months. All transplanted follicles have completed their growth cycles, and the hair has reached its full thickness, length, and texture. The transplanted hair is permanent and will continue to grow naturally for the rest of your life. Final comparison photographs are taken at your 12-month follow-up.

Hair washing instructions
Proper hair washing is critical to graft survival and healing. Follow these instructions carefully for each stage of recovery.
Do not wash or wet the transplanted area. Use saline spray only as directed.
First wash at the clinic or at home following our video guide. Apply the prescribed lotion to the recipient area, let it sit for 20 minutes to soften scabs, then rinse very gently with lukewarm water using a cup - no direct shower pressure.
Continue gentle washing daily. Use the medicated shampoo provided by the clinic. Apply with fingertips using minimal pressure. Let water run over the area rather than rubbing.
Gradually increase gentle fingertip pressure when washing to help remaining scabs detach. All scabs should be gone by day 10-14. You may resume using your regular mild shampoo.
Resume your normal hair-washing routine. Avoid harsh chemical shampoos or vigorous scrubbing of the transplanted area for the first month.
Sleeping position guide
Sleeping correctly after your hair transplant protects the newly placed grafts and reduces swelling. Here is what we recommend:
- ✓Nights 1-7: Sleep on your back at a 45-degree angle using 2-3 stacked pillows. A travel neck pillow prevents your head from rolling sideways.
- ✓Nights 8-14: You may begin sleeping at a lower elevation (1-2 pillows) but continue sleeping on your back. Avoid pressing the recipient area against the pillow.
- ✓Week 3+: You can return to your normal sleeping position. The grafts are securely anchored by this point.
Exercise restrictions timeline
Physical activity increases blood pressure and sweating, which can affect graft healing. Follow this timeline to safely return to exercise.

When to Contact the Clinic
While complications are rare, contact Zeeva Clinic immediately if you experience any of the following:
- Excessive bleeding that does not stop with gentle pressure after 15 minutes
- Signs of infection: increasing redness, warmth, pus, or foul smell from the donor or recipient area
- Fever above 101°F (38.3°C) that persists for more than 24 hours
- Severe swelling that spreads to the eyes or causes vision changes
- Allergic reaction to prescribed medications (rash, difficulty breathing, severe itching)
- A graft that appears to have been dislodged or pulled out accidentally

Frequently asked questions
Most patients with desk jobs can return to work within 5-7 days. If your work involves physical labour, heavy lifting, or outdoor sun exposure, you may need 2-3 weeks off. Many patients who work remotely return to their desks within 3-4 days. The small scabs in the recipient area are typically gone by day 10-14.
Shock loss is temporary and completely normal. The transplanted hair shafts shed within the first 2-6 weeks, but the follicle roots remain alive beneath the skin. New hair growth typically begins around month 3 and continues to thicken through month 12. The transplanted follicles are permanent.

The full result of a hair transplant becomes visible at 12-18 months post-surgery. You will begin seeing new growth around month 3, noticeable improvement by month 6, and approximately 80% of the final result by month 9. The last 20% of density and thickness develops between months 9 and 18.
You should wait at least 6 weeks before applying any chemical treatments (dye, bleach, keratin) to your hair. After 6 weeks, the grafts are fully secure and you can treat the transplanted hair exactly as you would your natural hair - it can be cut, coloured, styled, and combed normally.
In the first 48-72 hours, grafts are still settling into place. If you accidentally dislodge a graft (through scratching, rubbing, or trauma), contact the clinic immediately. After 5-7 days, the grafts are anchored securely and cannot be dislodged by normal activities like gentle washing. Note that shed hair shafts (shock loss) are not the same as lost grafts - the follicle remains intact.
